Stop by during Member Services hours!* We can have your membership card(s) ready in just a few minutes.

Feel free to complete the forms below ahead and bring them with you, or complete them when you visit. 

• Membership Agreement
• Health History Profile form (required for each adult member, age 14 and older)
• 3 Day Right to Cancel form
• Waiver of Medical Consent form
• Medical Consent form (we will help you obtain if needed, but you can get started right away)
• Authorization to Disclose form

Member Services hours: Monday through Friday, 8:30 a.m.- 6:30 p.m.; Friday, 8:30 a.m. - 5:30 p.m. and Saturday & Sunday, 8 a.m. - 3 p.m.

• Free personalized fitness consultation and orientation with one of our degreed and certified fitness coaches.
• Fitness area with Cardio and Strength training equipment including LifeFitness & FreeMotion; spacious free weight area
• Access to more than 70 group fitness classes offered in two beautiful studios and pools

• Aquatics Center including 25 meter lap pool, warm water pool and outdoor leisure pool opened seasonally

• 10% savings on Pavilion Spa services such as massages and facials
• Fully supplied locker rooms, complete with whirlpools, steam rooms, saunas, full-size day lockers and towel service provided.
