Community Grants

Now Accepting Applications

The McCullough-Hyde Foundation is excited to announce the launch of the Community Granting Program, as part of the expanded philanthropic mission of the Foundation. It is anticipated that more than $200,000 in funding will be available annually through this grant-making program.

The Community Granting Program is a philanthropic program that makes investments that address significant health needs of local communities. The Foundation will partner with and provide funding to local organizations that are leading the way to build healthier communities.

The McCullough-Hyde Foundation invites proposals from organizations for programs and projects that focus on education, prevention programs, awareness efforts, wellness opportunities and direct care in the following areas:

  • Chronic Diseases: Programs and projects that prevent, manage, and reduce the burden of chronic conditions including, but not limited to, heart disease, hypertension, stroke, and diabetes. Eligible initiatives may focus on education, early detection, care coordination, wellness promotion, and access to treatment and support services.
  • Healthy Behaviors: Programs and projects that promote and sustain healthy lifestyle choices, including, but not limited to, physical activity, nutrition, tobacco cessation, and preventive care. Eligible initiatives should be community-centered and designed to empower individuals and families to take control of their health through activities, education, engagement, and improved access to wellness opportunities that support active living, particularly among youth and families.
  • Mental and Behavioral Health: Programs and projects that improve access to and quality of mental health and substance use services, reduce stigma, and promote emotional well-being across all age groups. Eligible initiatives may include, but are not limited to, prevention, early intervention, treatment, recovery support, education, and outreach related to conditions including, but not limited to, anxiety, depression, and addiction.
  • Social Determinants of Health: Programs and projects that address the underlying social and economic conditions influencing health outcomes, including, but not limited to, access to healthcare, food, housing, transportation, education and employment. Eligible initiatives should aim to reduce barriers to health and wellness by improving the environments where people live, work, and play.

Eligibility

Organizations must be a 501(c)(3) or other not-for-profit organization (or have a tax-exempt fiscal partner) located in Butler County (OH), Franklin County (IN), Preble County (OH) or Union County (IN).

Funding Cycle and Application Deadlines

April 15, with notification in June
August 15, with notification in October
December 15, with notification in February

Application and Selection

The Community Grant Application link is located online at TriHealth.com/MHMHFoundation. The application process is comply contained within an online portal.* Grant applications will be reviewed and considered by the Community Grant Committee. The Community Granting Committee will submit a slate of selected grants to the Board of Trustees for final approval.
