Booking appointments are specific to the facility and the individual Outpatient Dietitian. Details are provided for each Outpatient Dietitian below.

Payment:** Some of the providers listed below do not accept insurance. Your insurance company may contribute to payment for education sessions, but some participants may need to cover the fee out of pocket. Please check with your insurance company before scheduling an appointment to determine availability of coverage.

How to Prepare for your Appointment

Please complete the Pre-Visit Questionnaire to help our team better prepare for your upcoming visit.
You are not required to bring anything to your education session. However, any documents that may be pertinent to the visit are encouraged.
A Food Diary can help us get a better understanding of your current dietary habits. We encourage you to bring this with you to your education session.
